The Know Your Numbers program is designed by HII to help you better understand your health and risk for disease. It’s important to us that you prioritize your health and to do this you must first know your health numbers. The Know Your Numbers program is established by HII and administered by QuadMed. All program requirements are at the direction of HII.
*If you have not already created your Wellness Online account (via MyChart), you must use a web browser (such as Chrome (preferred), Edge, or Safari) on your desktop or mobile device to access Wellness Online. After your account has been created, you can access Wellness Online through the MyChart or Virgin Pulse mobile app.

SIGN UP FOR A MYCHART ACCOUNT
A QuadMed MyChart account is required to access Wellness Online, where you will schedule your Know Your Numbers health screening and complete your online wellness profile.
step 1:
Visit myquadmed.com/hii and click the MyChart Sign Up button at the top of the page.
step 2:
Fill out all the information to confirm your identity and click SUBMIT.
step 3:
Check your email for a message from patientsupport@quadmedical.com and click the button in the email.
A health screening is one of two requirements needed to complete the Know Your Numbers program. You have three options for completing your biometric health screening:
At a screening event near your worksite
At the HII Family Health Center
At Shipyard Quick Care outside Gate 1 (Ingalls Only)
Appointments for biometric health screening events near your worksite and at the Family Health Center can be scheduled in MyChart using the following instructions.
INGALLS SHIPBUILDING EMPLOYEES ONLY: Ingalls Shipbuilding employees can complete their biometric health screening at Shipyard Quick Care during normal business hours – Monday through Friday from 8 a.m. to 4 p.m. No appointment is necessary. The blood draw is a fingerstick only. Shipyard Quick Care is located outside of Gate 1. For more information, call 228-935-4890.

Visit myquadmed.com/hii and use the buttons at the top of the page to log in to or sign up for MyChart.
Access Health Screenings in MyChart. Once logged in to MyChart, click on Menu.

Under Wellness, select Health Screenings.

On the Health Screening page, select Go in the Schedule Appointment box.

From the Calendar view, scroll to your preferred month using the arrow buttons on the top right or look for your desired screening location. CLICK ON YOUR PREFERRED TIME SLOT. To see ALL available dates, times and locations for a day, CLICK + MORE.

Review the event details and click SCHEDULE to make your appointment.

Once your appointment is scheduled, you will see it in the Schedule Appointment box on the Health Screenings page.
Please Note: You can cancel your appointment by selecting CANCEL at the bottom of the appointment detail.
Once logged in to MyChart, access Wellness Online from the shortcuts header at the top of the homepage or from the Wellness section of Your Menu.
For the 2023-2024 Know Your Numbers program, ALL participants must create a NEW account in Wellness Online.
To set up your NEW Wellness Online account this year for the first time, create your account using a web browser (such as Chrome (preferred), Edge or Safari) on your desktop or mobile device to access Wellness Online. After your account has been created, you can access Wellness Online through the MyChart or Virgin Pulse mobile apps.
step 3A:
Setup your Wellness Online account. If you have already setup your Wellness Online account, skip to step 4. Review and acknowledge the agreement page. Click NEXT.
step 3B:
Enter the requested information using your preferred email address. The time zone should be selected for the state that you reside in. Once complete, click CREATE MY ACCOUNT.
step 3C:
You are now ready to access your account at any time from your browser. Click on Take Me There and then click theX to close the pop-up.
step 3D:
Read and agree to the GINA/PHI notice. Click CONTINUE. Your new Wellness Online account is now setup.
step 4A:
Complete your Health Check Survey. If this is your first time in Wellness Online for the 2023-2024 program year, click START THE SURVEY to begin.
step 4B:
If you already have an account click on HEALTH and then SURVEYS to access your Health Check Survey.
step 4C:
Once complete, you will receive your health score and the results.

If you begin the Health Check Survey and are unable to finish, you can pick up where you left off the next time you sign in. All questions must be answered to complete the Health Check Survey and earn credit toward the Know Your Numbers program. Don’t forget the form! Download the Provider Health Screening Form below and take it with you to your provider. If you live in New York or California, please select the specific form for your state. All other states should use the All Other States Form.
You can check your progress in the Know Your Numbers program at any time through Wellness Online to see what requirements still need to be completed, or to confirm that you have completed the program.

Check your program completion status.
Hover over the HOME icon and then click on REWARDS. On the Rewards page, scroll down to the Know Your Numbers section.

The KYN program is one program made up of two actions. When both actions are complete, this section will read 1/1.
In the Complete Biometric Health Screening and Health Check Survey section, you will see a check mark next to the action you have completed. When both actions are completed, it will show 2/2 for Complete Biometric Health Screening and Health Check Survey. Please be sure to monitor your status frequently throughout the KYN program until both actions are complete.
step 4A:
View your health screening results
Hover over the HOME icon and then click on STATS. On the Stats page, scroll down to the Know Your Numbers section.
• Weight – Click here to see your weight and BMI
• Blood pressure – Click here to see your systolic and diastolic blood pressure readings
• Cholesterol – Click here to see your total cholesterol, LDL, HDL and Triglyceride readings
• A1C – Click here to see your A1C result
PLEASE NOTE: It can take up to 10 days FROM THE DATE OF RECEIPT for your information to be fully processed IN WELLNESS ONLINE.
note:
It may take up to two weeks after your completed health screening for the completion status to be reflected in Wellness Online.
