The Know Your Numbers program is designed by HII to help you better understand your health and risk for disease. It’s important to us that you prioritize your health and to do this you must first know your health numbers. The Know Your Numbers program is established by HII and administered by QuadMed. All program requirements are at the direction of HII.

SIGN UP FOR A MYCHART ACCOUNT
A QuadMed MyChart account is required to access Wellness Online, where you will schedule your Know Your Numbers health screening and complete your online wellness profile.
step 1:
Visit myquadmed.com/hii and click the MyChart Sign Up button at the top of the page.
step 2:
Fill out all the information to confirm your identity and click SUBMIT.
step 3:
Check your email for a message from patientsupport@quadmedical.com and click the button in the email.
A health screening is one of two requirements needed to complete the Know Your Numbers program. You have three options for completing your biometric health screening:
At a screening event near your worksite
At the HII Family Health Center
At Shipyard Quick Care outside Gate 1 (Ingalls Only)
Appointments for biometric health screening events near your worksite and at the Family Health Center can be scheduled in MyChart using the following instructions.
INGALLS SHIPBUILDING EMPLOYEES ONLY: Ingalls Shipbuilding employees can complete their biometric health screening at Shipyard Quick Care during normal business hours – Monday through Friday from 8 a.m. to 4 p.m. No appointment is necessary. The blood draw is a fingerstick only. Shipyard Quick Care is located outside of Gate 1. For more information, call 228-935-4890.

Visit myquadmed.com/hii and use the buttons at the top of the page to log in to or sign up for MyChart.
Access Health Screenings in MyChart. Once logged in to MyChart, click on Menu.

Under Wellness, select Health Screenings.

On the Health Screening page, select Go in the Schedule Appointment box.

From the Calendar view, scroll to your preferred month using the arrow buttons on the top right or look for your desired screening location. CLICK ON YOUR PREFERRED TIME SLOT. To see ALL available dates, times and locations for a day, CLICK + MORE.
step 5:
Review the event details and click SCHEDULE to make your appointment.

Once your appointment is scheduled, you will see it in the Schedule Appointment box on the Health Screenings page.
Please Note: You can cancel your appointment by selecting CANCEL at the bottom of the appointment detail.
Once logged in to MyChart, access Wellness Online from the shortcuts header at the top of the homepage or from the Wellness section of Your Menu.

On the Wellness Online homepage, LOCATE THE “KNOW YOUR NUMBERS PROGRAM” BOX in the middle of the page, then SELECT THE “COMPLETE PROFILE” ICON.

Review the information in the pop-up window and CLICK THE “COMPLETE THE ASSESSMENT NOW” button to begin.

If you begin the Health Check Survey and are unable to finish, you can pick up where you left off the next time you sign in. All questions must be answered to complete the Health Check Survey and earn credit toward the Know Your Numbers program. Don’t forget the form! Download the Provider Health Screening Form below and take it with you to your provider. If you live in New York or California, please select the specific form for your state. All other states should use the All Other States Form.
You can check your progress in the Know Your Numbers program at any time through Wellness Online to see what requirements still need to be completed, or to confirm that you have completed the program.

From the Wellness Online homepage, LOCATE THE “KNOW YOUR NUMBERS PROGRAM” BOX in the middle of the page. Your completion status will show with one of the three boxes shown below.
No Requirements Completed
The yellow box will say, “Not started yet”You will see a 0/2 in the Achievements section and no achievement badges will be listed.
One Requirement Complete
The yellow box will say, “Good work so far. Keep going! One more step to complete!” You will see a 1/2 in the Achievements section and one achievement badge will be listed.
Both Requirements Complete
The yellow box will say, “Great job! You are done!” You will see a checkmark in the Achievements section and both achievement badges will be listed.
step 4:
From the Wellness Online homepage, select the drop-down menu in the top left corner. Scroll down the menu and select one of the following items:
Weight & Biometrics to view your height, weight, BMI and blood pressure results.
Blood Panel to view your cholesterol and A1c results.
note:
It may take up to two weeks after your completed health screening for the completion status to be reflected in Wellness Online.
MyChart allows you to schedule and start a virtual visit with a QuadMed health care provider. It also provides additional features that help you to manage your health completely online.

QuadMed Everywhere will be available in your state beginning Sunday, October 1!
Scheduling will open on Sunday, October 1 and patient care will begin the following day on Monday, October 2. Check back in October to schedule your first appointment!
