SIGN UP FOR A MYCHART ACCOUNT

A QuadMed MyChart account is required to access Wellness Online, where you will schedule your Know Your Numbers health screening and complete your online wellness profile. 

step 1:

Visit myquadmed.com/hii and click the MyChart Sign Up button at the top of the page.

step 2:

Fill out all the information to confirm your identity and click SUBMIT.

step 3:

Check your email for a message from patientsupport@quadmedical.com and click the button in the email.

step 4:

Confirm your identity and click NEXT.

COMPLETE YOUR WELLNESS PROFILE

An online wellness profile is one of two requirements needed to complete the Know Your Numbers program. Your wellness profile is accessible through Wellness Online.

step 1:

Visit myquadmed.com/hii and use the buttons at the top of the page to log in to or sign up for MyChart.

step 2:

Once logged in to MyChart, access Wellness Online from the shortcuts header at the top of the homepage or from the Wellness section of Your Menu.

 

step 3A:

On the Wellness Online homepage, LOCATE THE “KNOW YOUR NUMBERS PROGRAM” BOX in the middle of the page, then SELECT THE “COMPLETE PROFILE” ICON.

step 3B:

Review the information in the pop-up window and CLICK THE “COMPLETE THE ASSESSMENT NOW” button to begin.

noteS:

If you begin the Wellness Profile and are unable to finish, you can pick up where you left off the next time you sign in. All questions must be answered to complete the wellness profile and earn credit toward the Know Your Numbers program. SCHEDULE YOUR HEALTH SCREENING

A health screening is one of two requirements needed to complete the Know Your Numbers program. You have three options for completing your health screening:

  • At a screening event near your worksite
  • At the HII Family Health Center
  • With your primary care provider (be sure to submit BOTH pages of the Provider Health Screening form)

Appointments for health screening events near your worksite and at the Family Health Center can be scheduled on Wellness Online using the following instructions. 

MISSION TECHNOLOGIES EMPLOYEES ONLY: Mission Technologies employees may complete their health screening via a Provider Health Screening form. Employees local to the NNS or Ingalls Shipbuilding locations may walk in to the FHC or an onsite event to complete their screenings. No appointment necessary.

Masks are required for all health screening appointments regardless of vaccination status. If you do not have a mask, one will be provided.

Once logged in to MyChart, access Wellness Online from the shortcuts header at the top of the homepage or from the Wellness section of Your Menu.

 

step 3A:

On the Wellness Online homepage, LOCATE THE “KNOW YOUR
NUMBERS PROGRAM – GET STARTED” SECTION in the
center of your homepage and CLICK ON THE “SCHEDULE
SCREENING” QUICK LINK in the center column.

step 3B:

From the Calendar view, scroll to your preferred month using the arrow buttons on the top right or look for your desired screening location. CLICK ON YOUR PREFERRED TIME SLOT. To see ALL available dates, times and locations for a day, CLICK + MORE.

step 3C:

Review the event details and click RESERVE to make your appointment.

step 3D:

The status will change to RESERVED when your appointment has been accepted.
Please Note: You can cancel your appointment by SELECTING CANCEL RESERVATION at the bottom of the reservation detail.

step 4:

Once your health screening appointment is made, it will be visible from the left side of your Wellness Online homepage
in the Upcoming Reservations section.

To view, cancel or reschedule your appointment, CLICK THE WHITE ARROW. From the overview, CLICK THE GRAY ARROW to view details and CLICK CANCEL RESERVATION to delete the appointment.

note:

Once your health screening appointment is made, it will be visible from the left side of your Wellness Online homepage in the Upcoming Reservations section. To view, cancel or reschedule your appointment, click the white arrow. From the overview, click the gray arrow to view details and click cancel to delete the appointment. 

COMPLETE PROVIDER FORM

Download the Provider Health Screening Form and take it with you to your provider. Both pages of the form must be filled out and signed prior to submitting. Once completed, submit both pages of the form to QuadMed. Forms can be submitted:

  • Fax to (414) 622-3802
  • Mail or drop off at your local health center

CHECK YOUR COMPLETION STATUS

You can check your progress in the Know Your Numbers program at any time through Wellness Online to see what requirements still need to be completed, or to confirm that you have completed the program.

step 1:

step 2:

Once logged in to MyChart, access Wellness Online from the shortcuts header at the top of the homepage or from the Wellness section of Your Menu.

 

step 3:

From the Wellness Online homepage, LOCATE THE “KNOW YOUR NUMBERS PROGRAM” BOX in the middle of the page. Your completion status will show with one of the three boxes shown below.

No Requirements Completed

The yellow box will say, “Not started yet”You will see a 0/2 in the Achievements section and no achievement badges will
be listed.

One Requirement Complete

The yellow box will say, “Good work so far. Keep going! One more step to complete!” You will see a 1/2 in the Achievements section and one achievement badge will be listed.

Both Requirements Complete

The yellow box will say, “Great job! You are done!” You will see a checkmark in the Achievements section and both achievement badges will be listed.

step 4:

From the Wellness Online homepage, select the drop-down menu in the top left corner. Scroll down the menu and select one of the following items:

  • Weight & Biometrics to view your height, weight, BMI and blood pressure results.

  • Blood Panel to view your cholesterol and A1c results.

note:

It may take up to two weeks after your completed health screening for the completion status to be reflected in Wellness Online.
