When you sign up for text messages from QuadMed, you expressly agree that you are authorizing us to send you messages through MyChart or other electronic means related to your relationship with QuadMed, including but not limited to updates relating to your visits, your MyChart account, one-time passcode, billing notifications, prescription reminders, and care management.
To the extent permitted by applicable law, you authorize us to contact you using automatic telephone dialing systems, artificial or prerecorded voice message systems, text messaging systems, and automated email systems in order to provide you with information, including information as referenced above and information about appointment and exam confirmations and reminders, wellness checkups, lab results, prescription notifications, home healthcare instructions, bill payment reminders, debt collection efforts and any other information and marketing messages. You understand you may withdraw your consent to receive text messages and calls by calling us at 888-417-1001. You can opt-out of SMS messages by texting STOP to respective short code. Your opt-out request will generate one final message confirming that you have been unsubscribed. You will no longer receive SMS messages from the short code you opted out from. If you want to join again, sign up by using your MyChart account, texting HELP to the short code for instructions, or contacting your QuadMed health center.
You understand that you are not required to agree to the electronic and telephone communications discussed above as a condition to our providing services to you.
