related to your relationship with QuadMed, including but not limited to updates
relating to your visits, your MyChart account, one-time passcode, billing notifications,
prescription reminders, and care management.
To the extent permitted by applicable law, you authorize us
to contact you using SMS text messaging, whether or not SMS text messaging is
conducted or deemed conducted via automatic telephone dialing or text messaging
systems, in order to provide you with information, including information
relating to your visits (appointment and exam confirmations and reminders, home
healthcare instructions, wellness checkups, lab results), your MyChart account,
one-time passcode, billing notifications, prescription reminders, and other
messages relating to your care. You understand you may withdraw your consent to
receive text messages and calls by calling us at 888-417-1001. You can opt-out
of SMS messages by texting STOP to the respective short code. Your opt-out
request will generate one final message confirming that you have been
unsubscribed. You will no longer receive SMS messages from the short code you
opted out from. If you want to join again, sign up by using your MyChart
account, texting HELP to the short code for instructions, or contacting your
QuadMed clinic.
You understand that you are not required to agree to the
electronic and telephone communications discussed above as a condition to our
providing services to you.
