How to request medical records

At QuadMed, we’ve streamlined the records request process to make it easier for you. With several convenient options available — including MyChart, digital forms and traditional forms — you can choose the method that works best for you.

Please select one of the following options based on your request or the type of records you’re seeking:

1. I am a Patient Requesting My Own Medical Records

2. I am Requesting Records for a Proxy or someone else

3. I am a Third Party Requesting Records

1. I am a Patient Requesting My Own Medical Records

If you are requesting your own medical records, we recommend using MyChart. However, there are other options available:

OPTION 1

Request Records via MyChart

Log in to MyChart to request your medical records. This option is for personal records only and cannot be used for proxies or accounts you manage.

To request your records:

Log in to your QuadMed MyChart account.
Click the Menu icon and select the Sharing Hub.
Choose Request formal copy of health record.
Complete the required fields and provide your consent for release of information.

Request Records via Sharecare

Click here to access the Sharecare request form for medical records requests.

For assistance with an existing request please call 800-560-3800, option 2.

OPTION 3

Submit a Request Form

Complete the QuadMed Authorization to Release Protected Health Information (PHI) Form (Versión en español) and submit it via one of the following methods:

Fax: Fax your completed form to 414.622.3809.
Email: Email your completed form to medicalrecords@ quadmedical.com.
Mail: Send your completed form to:

2. I am Requesting Records for a Proxy or someone else

If you are requesting medical records for a proxy, or someone you are authorized to represent (such as a child, dependent, or family member), we recommend using Sharecare.

OPTION 1

Request Records via Sharecare

Click here to access the Sharecare request form to submit your request for yourself or on behalf of a proxy.

For assistance with an existing request please call 800-560-3800, option 2.

OPTION 2

Submit a Request Form

Complete the QuadMed Authorization to Release Protected Health Information (PHI) Form (Versión en español) and submit it via one of the following methods:

Fax: Fax your completed form to 414.622.3809.
Email: Email your completed form to medicalrecords@quadmedical.com.
Mail: Send your completed form to:

3. I am a Third Party Requesting Records

Third parties (e.g., attorneys, insurance companies) requesting medical records can use Sharecare to submit their request.

General Information

Processing Time: Requests are usually completed within 7-10 business days. However, if your medical records are not available or complete, delays may occur. We will notify you if there is a delay.
Fees: In accordance with applicable state law, fees may be charged to cover the cost of preparing records for release.
Receiving Records: After payment, copies of your medical records will be mailed or available for pickup at the health center during regular business hours. A photo ID is required when picking up records in person.
