Your health center provides full primary care services with a strong focus on prevention. While we can treat you when you’re sick, it is our goal to keep you healthy and well for years to come.

Aches and pains are no match for our physical therapists. Our physical therapy sessions are always one-on-one with the same physical therapist, which allows you to achieve stronger results in fewer visits.
Get a variety of low-cost prescriptions right after your appointment. Many locations carry some discounted over-the-counter items such as aspirin, bandages, and vitamins, to help you save time and money.
We know health questions don’t always come up during business hours.
At no cost to you, the After-Hours Nurse Line connects you with a registered nurse when your QuadMed provider isn't available. You can get answers to questions about:
- Managing symptoms at home
- When to see a health care provider
- When to visit the emergency room
Get trusted health information and peace of mind when you need them most. Together, your QuadMed health center and the After-Hours Nurse Line ensure you're cared for around the clock, every day of the year.
Call 888-417-1001 to speak with a registered nurse when your QuadMed provider isn't available. The After-Hours Nurse Line is not accessible during your health center's operating hours.
QuadMed provides exclusive access to convenient, high-quality, affordable health care.
The health centers you have access to are operated by QuadMed, a third-party health and wellness vendor and nationwide leader in onsite health care with 30 years of experience in providing workplace health care.
QuadMed health centers are available to all associates (and their medically enrolled dependents) at Corporate, KIC, Photo Studio, Daycare and select Wisconsin Kohl’s store locations.
Service fees at any QuadMed health center are based on your health plan.
Selected medications will be available at the West Allis pharmacy and the Sussex and Lomira prescription centers. Many prescriptions can be filled up to a 90-day supply. Select pre-packaged medications are available at Hartford and West Bend health centers.
Your QuadMed provider can also prescribe medications to be filled at your local pharmacy.

QuadMed offers a range of basic lab services, including urinalysis, strep throat screening, pregnancy testing, and cholesterol, glucose, potassium and thyroid checks. You can also bring in orders from outside providers and have that lab work done at a health center.
No, the provider you visit is completely your choice. You can make any QuadMed location your primary source of health care and make the QuadMed provider your PCP, or you can continue to see your current PCP and use the health center whenever it is convenient for you.
Appointment times vary depending on symptoms. We have appointments available in 15-, 30-, 45- and 60-minute time frames to accommodate your needs.
No. Use of QuadMed health centers is completely voluntary and confidential.
MyChart allows you to schedule and start a virtual visit with a QuadMed health care provider. It also provides additional features that help you to manage your health completely online.
