TakeControl provides you with no-cost access to a team of educators who specialize in managing these conditions. Through one-on-one sessions, you’ll receive education and training to build the confidence and understanding you need to control your conditions and improve your health.

Health is never one-size-fits-all. TakeControl educators work directly with your primary care provider to develop a personalized care plan that combines medication with realistic lifestyle changes.
Our educators will teach you valuable self-management techniques that apply to your condition(s) and specific health needs, such as proper use of monitoring devices and strategies for overcoming day-to-day challenges. You will also learn lasting nutrition and exercise behaviors that can positively impact your health, and will receive ongoing support and accountability to help you achieve your goals.

As an added benefit, active members of TakeControl are eligible for significant discounts on condition-related supplies and medications by committing to the program and completing periodic health exams and sessions with our educators.
