During the COVID-19 pandemic, we are taking preventive measures to ensure the health and safety of our patients. At this time, in an effort to limit visits to the health center, we have taken a virtual-first approach to care. Virtual visits allow you to speak with a care provider for your health and wellness needs without having to visit the health center.
Virtual Visits Are:
You can schedule a virtual visit by calling your health center. Appointments can be made for a specific time during normal health center hours.
When your virtual visit begins via telephone, you will receive a phone call from the health center at the time of your scheduled appointment. After being checked in by the health center staff, you will speak to your provider.
Video visits can be accessed via your mobile phone, tablet or desktop device with a camera. You will also need an internet connection and a quiet, private place where you can speak with and hear the provider.
If video is necessary for your visit, a Zoom meeting link will be emailed to you, allowing you to connect to the provider through your web browser on a computer or through the Zoom app on a smartphone or tablet. Be sure to use the Zoom app rather than the web browser on your smartphone, tablet or mobile device.
You will receive a phone call from the health center up to 24 hours in advance of your scheduled appointment. A member of the health center staff may assist with checking you in before you speak with the provider. You may also check-in online for most appointments through your MyChart account.
Virtual visits enhanced through secure video conferencing are similar to a traditional office visit where you will see the provider and the provider will see you. If your visit results in a new prescription, your provider will be able to send it electronically to your preferred pharmacy.
Your provider may decide that you need additional care following a virtual visit. This may include follow-up in the health center, referral to a specialty provider, lab work or imaging. An After Visit Summary will be available for you to view through your MyChart account.

Complete the check-in process from home to save time before your virtual visit. You can verify your medical and insurance information, respond to questionnaires, and electronically sign documents. Once you have a visit scheduled, just login to MyChart, go to Visits tab, and click the ECHECK-IN button to sign consent forms.
In addition, you can give authorization for your healthcare providers to use Care Everywhere to share your medical record with other healthcare organizations when it’s needed for your care.

Start by visiting mychart.myquadmed.com and clicking “Sign Up Now.” If you received an activation code from the health center or on your patient bill, you can enter that and a few other pieces of information, which will allow MyChart to recognize you. If you do not have an activation code, just click “Don’t have an activation code? Sign up online.” on the right side panel. This method will require you to enter a few more pieces of information about yourself. Once that is complete, you will need to confirm the account setup from the automated email that is sent to the email address linked to your account.
