We regret to inform you that as of Thursday, December 1, 2022, QuadMed is no longer managing the Miller Health & Fitness Center located at 4400 W. State Street. As a result, access to care at the QuadMed shared locations in West Allis, Sussex and Lomira is also no longer available.
Ensuring the continuity of your care is important to us. If you continue care with the new health center vendor, they will be able to access your records through Care Everywhere. Alternatively, we can transfer a copy of your confidential medical records to another provider, or we can release them to you, or someone you designate, with your permission.
To request a copy of your medical records at any time, before or after the transition, visit myquadmed.com/how-to-request-your-medical-records to complete an Authorization to Release Information form. Please contact 888.417.1001 if you have any questions.
