Save time by signing up for MyChart and completing all the required steps prior to your appointment.

START HERE IF YOU ARE A NEW MYCHART USER

STEP 1

Create Your Account

Visit myquadmed.com to access to MyChart

Click SIGN UP NOW – If you have an activation code, enter it. If you don’t have an activation code follow the steps below.

Click SIGN UP ONLINE – Fill out all the information to confirm your identity and click SUBMIT

Check your email for a message from patientsupport@quadmedical.com and click the button in the email
Confirm your identity and click NEXT
You now have the ability to schedule and complete your virtual visit.

START HERE IF YOU ARE AN EXISTING MYCHART USER

STEP 2

Schedule Your Appointment

Visit myquadmed.com to access to MyChart

Log in with your user name and password.

If you don’t have a MyChart account, please return to STEP 1.

Click SCHEDULE AN APPOINTMENT

Select your visit reason.

Select VIRTUAL VISIT as your preferred visit type

Select the provider, date and time for your visit

STEP 3

Complete Pre-Visit Requirements

eCheck-in online prior to your visit
to update health information

eCheck-In must be completed before you can begin your visit.

STEP 4

Begin your Virtual Visit

Click BEGIN VIDEO VISIT

Click JOIN CALL

TIPS FOR A SUCCESSFUL VIRTUAL VISIT

Be sure to sign up for a QuadMed MyChart account.
Find a quiet, comfortable space where you won’t be interrupted.
Make sure you have strong Wi-Fi or wired internet connection.
If using a mobile phone, make sure you have a strong cell signal and your battery is charged.
Position your camera at eye level for the best interaction with your provider.
Choose a well-lit area with minimal background distractions.
