What is a PCMH?

The Patient-Centered Medical Home (PCMH) is a unique model of health care that prioritizes you as a patient and simplifies the way that you access and receive care. Through continuous education and support from a team that is dedicated to your health, you will learn to become an active participant in your care and will be empowered to make informed decisions to help you live a better, healthier life.

In addition to more direct access to care, we offer greater appointment availability with immediate visits for urgent needs. We also provide virtual visits and several options to communicate with your provider.

With longer appointment times, we have a better opportunity to listen to and address your concerns. We take the time to educate you so that you understand your health and how to manage it effectively.

Active involvement in your care

Our care providers partner with you to develop a care plan with achievable goals that fit your lifestyle. Changes are always made at your own pace and questions are welcomed – in fact, they’re encouraged.
