A. I voluntarily and of my own free will agree to participate as a model in a project for Quad/Graphics, Inc., Quad/Photo, Quad Creative Group, or a subsidiary or affiliate of the company (collectively, “Quad”). I agree and desire to do so for no fee, compensation or other payment of any kind. I hereby irrevocably grant and authorize Quad the worldwide right and license in perpetuity to use and publish my image, volunteering information and story, and any other personal information (including but not limited to any/all property, possessions or objects depicted either in whole, in part or implied, with or without my image) that is or may be created by photography or various mechanical and or electrical means of all descriptions in any manner Quad reasonably deems fit and for any reasonable purpose whatsoever. Further, I hereby grant and authorize Quad to copyright, publish, display, distribute, exhibit or otherwise make use of my image, volunteering information and story, and any other personal information which may be so created in whole or in part or in composite, modified, altered or distorted character or form for itself or its client for any lawful purpose.

B. I represent and warrant that I have ownership, license, intellectual property rights in and to the image(s), volunteer information and story, I submitted and that no third-party has any ownership, license, intellectual property rights in and to the image(s), volunteer information and story or any other aspect of my submission.

C. I hereby irrevocably waive any right that I may have to inspect and/or approve the finished product or any unfinished versions thereof, its context, use or any material used in conjunction therewith. I understand and agree that there is no guarantee that the image, volunteer information or story will be used, at any time in the future, in any manner or medium, for any purpose, and that I will not be notified if or when the image(s), volunteer information or story is used.

D. I understand and agree that Quad owns all worldwide right, title and interest to the copyright to any product within which my image and any other personal information is or may be incorporated.

E. In consent to the use of my image, volunteering information and story, and any other personal information, in the aforementioned manner, I expressly and irrevocably waive and release Quad and its employees, directors, representatives and agents from any and all claims and causes of action for or with respect to liability or damages, injury, liable, slander, loss, invasion of privacy, right of publicity, copyright infringement, defamation and any and all other claims, causes of action and liability by virtue of use, publication, alteration, optical illusion, or use in composite or other form whether intentionally or otherwise, that may occur or be produced in the taking or use of my image, volunteering information and story, and any other personal information, or in any processing.

By clicking the box labeled “(Optional) By clicking this box, I agree that I have read, understood and agreed to the terms of the Volunteer Photo and Story Release Agreement” on the QLife Volunteer Requirement submission, I warrant that I am of legal age and declare my acceptance and compliance with the entirety of this agreement. I agree that clicking that box is my electronic signature that authenticates this agreement and has the same force and effect as a manual signature.
